NDA/NDCL Athletic Hall of Fame Nomination Form
Nominees must have either participated in any of NDA or NDCL’s major interscholastic competitive sports as a player, coach, administrator, or in some other capacity which displays his or her interest and loyalty to NDA and/or NDCL. Candidates should be recognized as having had exceptional skill and/or dedication to their sport(s). Recommendations should include a summary of your candidate’s athletic achievements while at NDA or NDCL.
If your nominee has used his or her skills to enrich the community by volunteering as a coach and/or teacher, please be sure to include that information. Nominees must be 10 years removed from high school.
